Wolf in sheep's clothing: Uncovering how deadly bacteria trick the immune system

An outbreak of tuberculosis in the skid row area of downtown Los Angeles may have exposed up to 4,500 individuals to the bacterium that causes the deadly disease and has left federal officials scrambling to intervene.

The outbreak is occurring during winter, when homeless individuals are driven to crowded shelters, when influenza is peaking and when people's vitamin D levels, typically boosted by sunlight exposure, are low. A new UCLA study offers critical insight into how various bacteria may manipulate such factors to their advantage.

In a study published online Feb. 28 in the journal Science, UCLA researchers demonstrate that certain cunning bacteria ? including the type that causes tuberculosis ? can pretend to be viruses when infecting humans, allowing them to hijack the body's immune response so that they can hide out, unhindered, inside our cells. The findings may also help explain how viral infections like the flu make us more susceptible to subsequent bacterial infections such as pneumonia.

The study is particularly relevant to tuberculosis, which kills 1.4 million people worldwide each year. In the case of the recent Los Angeles outbreak, the findings could provide clues as to how the flu and a lack of vitamin D may have given the tuberculosis bacterium an edge.

"With 8.7 million in the world falling ill with tuberculosis each year, a better understanding of how these bacteria avoid our immune system could lead to new ways to fight them and to better, more targeted treatments," said senior author Dr. Robert L. Modlin, chief of dermatology at the David Geffen School of Medicine at UCLA and a professor of microbiology, immunology and molecular genetics in the UCLA Division of Life Sciences.

The protection our immune system provides against bacteria-based diseases and infections depends on the critical response of T cells ? white blood cells that play a central role in fighting infections ? and in particular on the release of a protein called interferon-gamma. Interferon-gamma utilizes the vitamin D hormone to alert and activate cells to destroy invading bacteria.

The research team found that bacteria can pretend to be viruses, triggering the immune system to launch an attack with a different protein, called interferon-beta, which is designed to fight viruses, not bacteria. Not only is interferon-beta ineffective against bacteria, but it can also block the action of interferon-gamma, to the advantage of bacteria. Further, if a real virus were to infect the body, triggering interferon-beta, it would divert the attention of the immune response, preventing an attack on the bacterial invader. The researchers say this may explain why the flu can lead to a more serious bacterial-based infection like pneumonia.

"Like a wolf in sheep's clothing, the bacteria can fool the immune system into launching an attack against the wrong type of infection, thus weakening the response against the bacteria," said first author Rosane M. B. Teles, a researcher in the dermatology division at the Geffen School of Medicine.

For the study, the team examined the mechanisms by which the virus-fighting interferon-beta protein suppresses the interferon-gamma defense response to bacterial infections, tricking the immune system into making the wrong defense choices.

The researchers studied leprosy as a model and then applied what they learned to understand tuberculosis, given that leprosy and tuberculosis are caused by related bacteria. Modlin noted that leprosy is an outstanding model for studying immune mechanisms in host defense since it presents as a clinical spectrum that correlates with the level and type of immune response of the pathogen.

The scientists first compared the genetic expression of the virus-fighting interferon-beta protein and the bacteria-fighting interferon-gamma protein in skin lesions from leprosy patients. They found that interferon-gamma was expressed in patients with the milder form of the disease and that interferon-beta was significantly increased in those with the more serious, progressive form of leprosy.

The researchers then compared the genes triggered by interferon-beta in these leprosy skin lesions with those found by two other groups of investigators in the blood of tuberculosis patients. Remarkably, there was a significant overlap. The interferon-beta genes were more frequent in both the skin lesions of leprosy patients with extensive disease and the blood of tuberculosis patients with more severe disease.

"We found this common interferon-beta gene pattern correlated with the greater extent of disease in both leprosy and tuberculosis, which are two very distinct diseases," Teles said.

Previous work by the UCLA team demonstrated that the interferon-gamma defense pathway relies on a specific mechanism involving vitamin D, a natural hormone that plays an essential role in the body's fight against infections. The current study found that interferon-beta suppressed elements involved in the interferon-gamma?triggered vitamin D pathway, preventing the immune system from killing the bacteria.

"The study raises the possibility that a decrease or increase of one of these two interferon proteins could shift the balance from mild to more serious disease," Modlin said. "We may find that therapeutic interventions to block or enhance specific interferon responses may be an effective strategy to alter the balance in favor of protection against bacterial diseases."

The new findings may indicate why, in winter, Los Angeles skid row residents are at an added disadvantage in dealing with tuberculosis ? for at least three reasons. First, because of colder weather at night, indigent homeless people tend to stay in shelters, where they live in close proximity with others, facilitating the spread of the infection. Second, due to the seasonal rise in influenza, the body's immune system could be diverted by the flu virus to produce interferon-beta, blocking an effective immune response to the tuberculosis bacteria. And finally, the drop in vitamin D levels associated with a decrease in exposure to sunlight during the winter months could diminish the ability of individuals' immune systems to kill the tuberculosis bacteria.

"With TB on the rise, this scenario could play out not only in cities in the United States but all over the world," Modlin said. "We hope that our findings may provide insight into harnessing new methods to combat TB and other bacterial infections as well."

Modlin noted that 8.7 million become ill with tuberculosis each year, and 1.4 million die from the disease. He added that an increase or decrease in one of the two interferon proteins could help explain why some people may be more resilient against or susceptible to the infection or have a more serious course of the disease.

The next step, according to Teles, is to further understand the mechanisms that bacterial pathogens use to activate interferon-beta and how bacteria can manipulate the immune system to block the potent interferon-gamma host antimicrobial responses in human infections.

Conservatives humbled in vote as anti-EU party surges

EASTLEIGH (Reuters) - Prime Minister David Cameron's ruling Conservative party was humiliated in an election in England on Friday after it was defeated by a scandal-ridden coalition partner and pushed into third place by an anti-EU party.

Cameron and his party had hoped to come second or even win the parliamentary election in Eastleigh, but were pushed into third place by the UK Independence Party (UKIP), a party that advocates taking Britain out of the European Union and strongly opposes immigration.

The result, which saw the Liberal Democrat party take first place, will pile pressure on Cameron from disgruntled MPs within his own party who fret he may not be able to lead them to victory in a 2015 general election since, to do so, he will have to win parliamentary seats like Eastleigh.

One senior party figure and a former leadership candidate, David Davis, warned before the vote that "a crisis" would ensue if the Conservatives were beaten into third place by UKIP.

The result was an important symbolic victory for the Lib Dems, however, who have been hit by a sex and perjury scandal, and for Nick Clegg, the deputy prime minister and party leader, who has seen his leadership come under pressure in recent weeks.

The Lib Dems polled 13,342 votes, UKIP 11,571 votes, the Conservatives 10,559 votes and the Labour party 4,088 votes. That means UKIP took almost 28 percent of the vote, one of their best ever results in a British parliamentary election to date.

Grant Shapps, the Conservative party chairman, denied the outcome was "a crisis" for Cameron, saying it was virtually unheard of for a governing party to win a new seat mid-term.

Diane James, UKIP's candidate, said the result was "a humongous political shock", while Nigel Farage, UKIP's leader, said the result showed Britons were weary of mainstream politics.

"It just goes to show that the UKIP message is really really resonating with voters," he told Reuters. "The EU and immigration are the same debate and that's the message the British public are now beginning to understand."

Farage's party has siphoned off support from Cameron's Conservatives by attacking EU bureaucracy and immigration from eastern Europe, tapping into what he says is a widespread feeling that mainstream parties have ignored voters' concerns.

PROTEST VOTE

The Eastleigh vote does not truly reflect national sentiment - the opposition Labour party leads in the polls nationally with the Lib Dems trailing in third or fourth place.

The election came at a time when Clegg's leadership of his party, without whom the rightist Conservatives cannot govern or pass legislation, faces intense pressure.

A sex scandal that has ensnared Chris Rennard, his party's former chief executive, has raised difficult questions for Clegg about what he knew about the imbroglio and when and how he dealt with it, amid accusations of a cover-up.

Rennard strongly denies the accusations, which centre on allegations of sexual misconduct towards female party workers, while Clegg says he was only previously aware of "non-specific" and "anonymous" allegations which could not be acted upon.

"Tonight is a great night for the Lib Dems nationally. A strong signal of support for Nick Clegg, an affirmation of our role working in the national interest within the coalition and a huge boost to our party's mission in government," said Mike Thornton, the winning Lib Dem candidate.

But most voters interviewed by Reuters on Thursday said they were unfazed by the sex scandal and viewed the vote as an opportunity to vent their frustration at politicians.

"You're left with one party with no policies, another party with a person standing against the leader's policies, and the other party is a pack of liars. So you say UKIP, let's have them in, and then these politicians may shape their minds up and start contacting the electorate rather than playing games in Westminster," said Noel Boulding, 72.

FAA looking into safety of sky-high Harlem Shake dance

NEW YORK (Reuters) - A sky-high video of the Internet dance craze Harlem Shake that went viral on YouTube has captured the attention of aviation officials who said on Friday they are looking into the event over safety concerns.

In the video called, "Harlem Shake Frontier Flight 157," members of the ultimate Frisbee team from Colorado College and other passengers are shown frenetically dancing in the aisle and in front of seats last month on the packed Frontier Airlines flight from Colorado Springs to San Diego, California.

"If we find any evidence of a violation of Federal Aviation Regulations, then we would pursue it," FAA spokesman Allen Kenitzer said in an email.

The song-and-dance craze typically begins with one dancer who is quickly joined by others, some of whom are in costume.

Frontier Airlines defended its decision to allow the college students to dance on board the flight thousands of feet over the Grand Canyon in Arizona.

"We have a policy not to comment on things that are under investigation by the FAA, but I can tell you that all safety measures were followed and the seatbelt sign was off," Frontier Airlines spokeswoman Kate O'Malley said in an email.

Frontier crew members had taped their own version of the dance a few weeks earlier, though on firmer ground inside an air terminal.

The college students said they had come up with the idea a few days before the flight. They had asked the flight staff for permission before beginning the dance and were shown how to use the public address system.

"When we got to the airport we were kind of ready and had brought costumes and stuff," Matt Zelin, a sophomore at the college, told the college newspaper, The Catalyst.

"Obviously I hope that this whole situation is solved with the FAA ...," Zelin added. "I don't see there being any reason why this should cause any trouble. We asked the staff and they said it was safe."

Discovery on animal memory opens doors to research on memory impairment diseases

If you ask a rat whether it knows how it came to acquire a certain coveted piece of chocolate, Indiana University neuroscientists conclude, the answer is a resounding, "Yes." A study newly published in the journal Current Biology offers the first evidence of source memory in a nonhuman animal.

The findings have "fascinating implications," said principal investigator Jonathon Crystal, both in evolutionary terms and for future research into the biological underpinnings of memory, as well as the treatment of diseases marked by memory failure such as Alzheimer's, Parkinson's and Huntington's, or disorders such as schizophrenia, PTSD and depression.

The study further opens up the possibility of creating animal models of memory disorders.

"Researchers can now study in animals what was once thought an exclusively human domain," said Crystal, professor in the Department of Psychological and Brain Sciences in the College of Arts and Sciences. "If you can export types of behaviors such as source memory failures to transgenic animal models, you have the ability to produce preclinical models for the treatment of diseases such as Alzheimer's."

Of the various forms of memory identified by scientists, some have long been considered distinctively human. Among these is source memory. When someone retells a joke to the person who told it to him, it is an everyday example of source memory failure. The person telling the joke forgot the source of the information -- how he acquired it -- though not the information he was told. People combine source information to construct memories of discrete events and to distinguish one event or episode from another.

Nonhuman animals, by contrast, have been thought to have limited forms of memory, acquired through conditioning and repetition, habits rather than conscious memories. The kind of memory failures most devastating to those directly affected by Alzheimer's have typically been considered beyond the scope of nonhuman minds.

The study owes much to another quality these rodents share with humans: They love chocolate. "There's no amount of chocolate you can give to a rat which will stop it from eating more chocolate," Crystal said.

Using chocolate as bait in an eight-arm radial maze, the study was made up of a series of five experiments that together provide converging evidence. In the first two, in order to obtain the chocolate, the rats needed to remember the source by which they acquired it, whether they were placed near the trough containing the chocolate or had to run on their own to get there. Using different mazes helped rule out the possibility that overlearned cues from a particular maze led to the positive results.

A third experiment showed that the rats' source memory, the means by which it retrieved the chocolate, lasted for a week rather than the one day that other, more ordinary forms of memory last. This provided converging evidence, said Crystal, that the rats were relying on source memory insofar as source memories decay more slowly than other memory systems.

In the fourth experiment, the rats could obtain the chocolate when the researcher placed them at the trough. The rats remembered this rule, too. Finally, in the fifth experiment, researchers temporarily disabled the rats' hippocampus, the brain region thought to be crucial for accurate source memory. If the task requires source memory, inactivating that area would impair the ability to remember source information, which it did.

"What we're trying to do is to develop behavioral approaches with rodents that tap into those types of memory systems," Crystal said. "This study is the demonstration, the proof of the concept that source memory exists in animals. But the mechanism that supports it is open. We're now interested in working out the sub-areas of the hippocampus that are involved in episodic memory, testing hypotheses about different regions being involved in short-term and long-term episodic memory, working out the neuroanatomical pathways."

Woman who smoked through hole in throat dies

LOS ANGELES (AP) ? A woman who smoked a cigarette through a hole in her throat to illustrate her struggle with nicotine addiction in a California public service advertisement has died of cancer, health officials and her family said Wednesday.

Debi Austin died Feb. 22 at Valley Presbyterian Hospital in Van Nuys, according to family friend and spokesman Jim Walker. She was 62.

Austin first appeared on television in 1996, telling viewers she began smoking at age 13 and could never quit. In a quiet, halting rasp, Austin told the camera, "They say nicotine isn't addictive," before inhaling from a lit cigarette held to a hole in her throat.

"How can they say that?" Austin asked viewers, as cigarette smoke wafted from the hole.

Called a stoma, the hole in her throat allowed her to breathe after her larynx was removed at age 42.

The TV spot was "the most-recognized and talked about California tobacco control ad," according to the state health department.

"Debi was a pioneer in the fight against tobacco and showed tremendous courage by sharing her story to educate Californians on the dangers of smoking," said Dr. Ron Chapman, who heads the health department. "She was an inspiration for Californians to quit smoking and also influenced countless others not to start."

Four months after the ad, Austin quit smoking ? halting a two- to three-pack-a-day habit. She fought various forms of cancer for the rest of her life. She starred in other ads and spent the rest of her life advocating against the use of tobacco.

"True to Debi's spirit, she was a fighter to the end and leaves a big hole in our hearts and lives. Debi will be remembered fondly by those who love her to be caring, courageous, very funny and always there to offer advice or lend a hand," the family's statement said.
